Most merciful Jesus, the consolation and Salvation of all who put their trust in Thee, we humbly beseech Thee, by Thy most bitter Passion, grant recovery of health to Thy servant (Name), provided this be for his (her) soul’s welfare, that with us he (she) may praise and magnify Thy holy name. But if it be Thy holy will to call him (her) out of this world straighten him (her) in his (her) last hour. Grant him (her) a peaceful death and bring him (her) to life everlasting. Amen.
